Put simply, bone bruises cause pain because the outer layer of the bone is damaged. When this layer is injured, inflammation and swelling occur, resulting in pain. As inflammation and swelling increase, nearby joints can become stiff, making movement and weight-bearing difficult or painful. 

Because bone bruising can be caused by direct contact with a hard object or surface, such as during a fall or car accident, associated pain can also be caused by bone chipping at the impact area.
