What Is Trigeminal Neuralgia?

Trigeminal neuralgia affects the trigeminal nerve, an essential cranial nerve providing sensory and muscle impulses to many parts of the head and face. This nerve starts near the top of the ear and then branches out into three sections towards the eye, cheek, and jaw. 

Trigeminal neuralgia causes electrifying, painful signals to shoot down one or more nerve branches. While there are two trigeminal nerves, one on each side of the face, pain normally affects only one side (unilateral).

It is a rare condition that can occur at any stage of life, but trigeminal neuralgia is more common in adults, especially in women over 50. Those diagnosed will face many daily struggles where the simplest of activities, such as eating, brushing teeth, or engaging in casual conversation, or even the weather, may trigger episodes of electrifying pain. For some people, even environmental factors such as the temperature or strength of the wind can lead to discomfort.

The frequency, intensity, and duration of trigeminal neuralgia pain episodes will depend on the underlying cause. However, it’s important to note that this painful condition is likely to progressively worsen if it is left untreated. We’ll look at this condition’s pathophysiology and underlying causes in the next section.

What Causes Pain From Trigeminal Neuralgia?

Trigeminal neuralgia is typically triggered by factors that cause nerve dysfunction, such as nerve damage, irritation, impingement, or other forms of nerve impairment. 

The most common cause is trigeminal nerve compression, typically by a large blood vessel or growth near the nerve root. This then causes nerve impingement that leads to its dysfunction and generation of inappropriate pain signals. Additionally, trauma to the head or face, even certain dental procedures, can directly damage the nerve and cause it to malfunction. 

While rare, trigeminal neuralgia may also stem from underlying neurological conditions such as multiple sclerosis, scleroderma, or shingles. Furthermore, if the tissues around the trigeminal nerve become significantly swollen due to infection or inflammation, it might irritate the nerve and cause it to become impaired. 

In some cases, trigeminal neuralgia may spontaneously appear, with doctors and professionals unable to find any evidence to pinpoint its underlying cause. When the root cause remains unknown, it is identified as “idiopathic trigeminal neuralgia.” 

Some studies hypothesize that in cases of idiopathic trigeminal neuralgia, the protective myelin sheath, a critical component of nerve structure, has dissolved. The myelin sheath can then not protect the nerve or support normal electrical conductivity, which may trigger the nerve to spontaneously malfunction and send improper nerve messaging.
